You must be a IIA of ILmember in good standing.

Fax a Brokerage Agreement along with a copy of your agency Insurance license and your E&O declarations page (with a minimum limit of $1,000,000) to Tami Hubbell at (217) 391-1156 or email to thubbell@iiaofillinois.org. Tami can be reached at (800) 628-6436, ext. 3016. Upon receipt you are approved to use the program. YOU WILL NOT BE NOTIFIED. You may also send a submission with your Brokerage Agreement etal.

Simple 5 step Submission Process:

Verify eligibility by reviewing the class code list and Underwriting Guidelines.

Fax the completed Acord 130 to AIS at (217) 391-1156 or email to thubbell@iiaofillinois.org. (Include the Inclusion or Exclusion form, if applicable).

Anticipate a faxed quote within 24 hours.

Fax back an acceptance to Agency Resources, Inc. at
(973) 261-9202.

Anticipate a faxed binder within 24 hours. The carriers will bill your client directly... And you're done!
Important! A complete Acord 130 application
improves response time.
